AMIGOS Paperwork

This is the first set of paperwork that includes: PART 1: consent and release forms, release of health information, standards of personal conduct form, letter of health disclosure, confidential health form 1; PART 2: confidential health form 2, and financial assistance application. Turn part 1 in to Cherie Andreassen on November 12, 2011 and part 2 on January 8, 2012.

Online Application

All volunteers must complete an online application for our International Office by November 12, 2011. This if different than the preliminary application you filled out to get an interview.
